If your project involves Animal Experimentation, Microorganisms or Human Subjects you must make a
Request for Ethics Ruling
You must make the request by January 23, 2025.
Human Subjects
If your project involves human participants EVERY participant must consent. Use these forms:
Editable consent form – fill in the editable boxes in the PDF then print the form for signature.
Blank consent form – boxes must be filled in on a printed version and signed.
Example form – a filled human consent example form to show what is expected.
